Luckily we’re equipped with the vestibulooccular reflexes to help us fixate on objects. Here you’re seeing the dolls eye reflex being attempted… Turning your head to the right should activate the ipsilateral (same side) medial rectus muscle but inhibit the contralateral side. Simultaneously it should also active the lateral rectus in the contralateral eye but inhibit the same muscle on the ipsilateral side. Unfortunately, in patients like this with a brain stem injury, the dolls reflex is absent. #doctor #nurse #nursesoftiktok #doctorsoftiktok #mbbs #medico #ukmla #usmle #osce #medicalquiz #diagnosis #medstudent #medicalschool #physicianassociate #premed #LearnOnTikTok #paces

This is VERY common. Here you’re seeing huge distension of the leg veins. Veins are “large capacitance vessels” (they can distend) and are usually full of valves to keep blood flowing towards the heart. Remember that bloods flows from the heart -> leg arteries -> tissue bed -> deep or superficial (e.g saphenous) veins -> iliac veins/inferior vena cava An upstream obstruction (injuries/lymphoedema/masses/previous blood clots, etc) or damage to the valves causes blood to back up, causing tortuous vessels called varicose veins. Treatment options include: ✅ Compression bandaging ✅ Sclerotherapy ✅ Laser therapy ✅ Surgical striping #doctor #nurse #nursesoftiktok #doctorsoftiktok #mbbs #medico #ukmla #usmle #osce #medicalquiz #diagnosis #medstudent #medicalschool #physicianassociate #premed #LearnOnTikTok #paces

They’re pretty obviously little Spiders! Secondly, where are we? Take a look towards the back and we’ll see the characteristic features of the tympanic membrane (ear drum) Bugs in the ear may be felt as itching, pain or unusual sounds! Small insects and arachnids like this can either be coaxed/shaken out, flushed out (e.g baby oil/vegetable oil) and/or removed with forceps! #doctor #nurse #nursesoftiktok #doctorsoftiktok #mbbs #medico #ukmla #usmle #osce #medicalquiz #diagnosis #medstudent #medicalschool #physicianassociate #premed #LearnOnTikTok #paces
🔥See answer below ✅ Get updated when the answer is released by commenting! 🔒 Follow me Dr Matt Barrett @dr_mattbarrett to study medicine smarter! —— ⚠️ Spoiler alert: subchondral recession Here we can see a cardinal sign of respiratory distress… substernal/subchobdral (and intercostal recession). On inspiration, the rib cage is moving inwards rather than outwards due to high negative pressure. We can see this as prominence of the ribs and lower costal margin with each breath. In someone of this age we should be thinking of some important differentials… 🔒 Think of respiratory diseases that affect children, by anatomical location (roughly proximal to distal)… Epiglottitis -> anaphylaxis -> croup -> bronchiolitis -> asthma 💡 In adults this is commonly associated with COPD and is called Hoover sign! #doctor #nurse #mbbs #medico #osce #medicalquiz #medstudent

The cause remains poorly understood but is characterised by dissolution of Bowman’s layer in the cornea, leading to an outward bulge. It’s usually bilateral and causes blurred vision, flaring and sensitivity to light. The usual treatment is with glasses/contact lenses but corneal transplantation may be need in severe cases. #doctor #nurse #nursesoftiktok #doctorsoftiktok #mbbs #medico #ukmla #usmle #osce #medicalquiz #diagnosis #medstudent #medicalschool #physicianassociate #premed #LearnOnTikTok #paces
